Connecting Seniors with Value-Based Health Care
GREENVILLE, SC (Sept. 14, 2017) Improving the health and wellbeing of seniors on Medicare is the goal of recent startup, Health Care Advantage. By connecting patients with doctors dedicated to seniors, Health Care Advantage helps seniors experience more time with their providers and better coordination of care.
In today’s health care environment, there is a shift away from the existing volume-based health care system toward a model of value-based care. Medical professionals are increasingly rewarded for improving health outcomes, not just the number of services and procedures provided.
Physician groups throughout Greenville have been adopting this improved model of care, with a focus on serving seniors on Medicare. Unfortunately, many individuals in the area don’t know they have the option to switch to a doctor dedicated to seniors.
“We started Health Care Advantage because we wanted seniors to have a choice in their health care experiences, and to feel comfortable exploring their options,” said Matt Tucker, president of Health Care Advantage. “Through our partnerships with local Greenville provider groups, we help seniors in this community find doctors who are committed to helping them get healthier.”
Currently, facilities are available throughout Greer, Anderson, Orchard Park and Powdersville. These locations offer a range of convenience services including medication coordination, prescription cost management, dedicated care teams and senior wellness programs.
Because they are focused on improving the health outcomes of their patients, these groups are able to offer longer, more frequent appointment times and shorter wait times at the doctor’s office, often without costing the patient more.
Health Care Advantage encourages seniors to schedule a free visit at any of its partner facilities to learn more. During these appointments, seniors can tour the locations, meet the doctors and determine if value-based care is right for them.
“We are excited to be a part of this shift toward Medicare’s value-based model of care,” said Tucker. “Seniors deserve to be heard, understood and respected – and to leave the doctor’s office feeling comfortable with their care plan. That’s what they get at our partner physician practices.”
